Treatment of a Lip Protrusion Patient Using Lingual Orthodontic Appliance with Long Lever-Arm and Skeletal Anchorage

Abstract
27-year-old woman came to the clinic. Her chief complaint were lip protrusion and lower anterior teeth crowding. She wanted a lingual orthodontic appliance on the maxilla and a labial appliance on the mandible. Because she had lip protrusion with large overjet, premolar extraction treatment was selected. The retraction wire with a long lever-arm and skeletal anchorage system was applied for torque and vertical control of upper anterior teeth. Moreover, additional over-torque was applied on maxillary anterior teeth during making set-up model to prevent excessive torque loss. After orthodontic treatment, the appearance of the lip protrusion was improved. Angle Class I molar and canine relationship were accomplished. The large overjet of anteriors were improved without excessive torque loss of upper anteriors.
